● Nursing Service:

 Service available for those with Nursing Insurance:
 A system that provides a comprehensive medical service for those who need nursing and want (if possible) to live in their home and lead an independent life. To judge if this service is available to you, first you will need to apply for Primary Nursing Care Requirement Authorisation.

● Procedure for Nursing Insurance

 ◆ From applying to authorisation
 For Nursing Insurance Service for bedridden people or those with dementia etc, you will need to apply for primary nursing care. The results of this application will determine the main support (level one or two) and the primary nursing care level (one to five).

◎ Application
 This is to judge whether nursing is necessary and if so, how much nursing is needed.
 For application, bring the nursing insurance certificate and the person’s seal to the Town Hall’s Health and Welfare Division.
※ Someone from the local support center, home nursing support or a nursing insurance representative etc can also be used instead of the actual person or the person’s family.
            ↓
◎ Home Visit Enquiry
 Staff from the Town Hall, Health and Welfare Association and/ or Komono Kousei Hospital will visit you in your home to do an interview. This will include examining mental circumstances and medical care required. Also, in particular nursing care levels etc will also be discussed.
※ The Family Doctor will also be asked for their written opinion.
If you have no Family Doctor, please inform the relevant Town Hall Division.
            ↓
◎ Primary Decision
 The results are gathered and entered into the computer. This is the draft for the secondary decision.
            ↓
◎ Secondary Decision
 Once the Primary Decision results and the home visit information are gathered, the doctor and the Nursing Care Review Board will make the final decision.
            ↓
◎ Notification of the Result
 Depending on the results (main support (level one or two) and the primary nursing care level (one to five)), a nursing service programme will be made.
※ Typically, primary nursing care is reviewed every six to 12 months (maximum 24 months).
Also, if circumstances change dramatically suddenly, then you can request an immediate change, without waiting for a review date.

◆ Applying for Nursing Insurance Service
◎ Over 65(First Insured Person)
 ・For those who are bedridden or have dementia etc, who need help with bathing, toilet and food etc.
 ・For those who need help with housework and dressing themselves etc
◎ Between 40 and 60 ( Second Insured Person )
 ・For those who have presenile dementia and cerebrovasular disease etc and need nursing aid.
Non-Insured: Cannot apply for Nursing Insurance
Main Support 1 or 2: Can use home help service
Primary Nursing Care one to five:Can use both home help service and facilities
*The Nursing Care Level is based on nursing time and not severity of disability
*When using the nursing service, you will bear 10% of the cost.
*If you do not agree with the authorisation results, please direct your dissatisfaction to the Mie Prefecture Nursing Insurance Association.

 ◆ Entering the Facility
・Welfare Facility for the Elderly A place where daily assistance and movement training etc is given.
・Rehabilitation Facility for the Elderly The focus is on daily movement training for symptoms and medical care etc.
・Medical Treatment is in a Hospital Environment Once medical treatment is completed, for those who need long time recuperation, this is a place to get doctors advice as well as daily movement training.
 Aimed at those who are not enrolled in the Nursing care insurance system for primary nursing care, an elderly welfare service has been set up. The details are as follows:

 ◆ Short Term Aid for Patients in their homes
 This is a service for elderly people who are not insured for nursing to go into temporary homes in order to provide help to carers to take a rest from caring or during important family ceremonies. This service has a cost.

 ◆ Diaper Provision Service ※
 For bedridden elderly people or those with severe dementia who use diapers all the time, a maximum amount of 7,500 yen per month in tickets can be provided.

 ◆ Everyday Life Tools Provisions ※
 A service for those over 65 living alone that provides fire alarms, sprinkling systems and electromagnetic cookers.
   * This service requires a cost.

 ◆ Daily Life Services ※
 For elderly people living alone over 65, a morning and evening food supply service is available.
   * This service requires a cost.

 ◆ Emergency Call Equipment ※
 A device given to those living alone over 65 who have a bad illness, so that they can contact someone in case of an emergency.

 ◆ Area Support Center
 In order for elderly people to continue to live in their familiar area, and in order to prevent nursing care for as long as possible, there are various services available. For this to happen, maintenance of good health, health care, welfare and stable living circumstances are important. For this reason there are public health specialists, welfare practitioners and care managers etc.

 A service that will help you plan for both Main Support (prevention payment) and specific Payments (prevention of nursing).

 Care Support Specialists, Family
